What are donations used for?

Seasons Hospice Foundation provides funding for the extraordinary needs of our patients and families that fall outside the traditional Medicare Hospice Benefit. Funding interventions can range from fulfilling a final wish for a quiet anniversary celebration, to helping pay utilitiy bills or covering the travel costs to bring a distant family member to a patient's side. Specific programs include our Making Memories Program, Special Bereavement Programs, Special Needs of all Hospice Patients including Pediatric Hospice Patients, as well as educational programs in the communities we serve.

Where is Seasons Foundation located and whom does it serve?

Seasons Hospice Foundation carries out its mission through patient interventions and education in the communities that are served by Seasons Hospice & Palliative Care across the United States. Seasons Foundation activities are supported and administered by both a National Team, located in Rosemont Illinois, and local Foundation Committees made up of hospice professionals, front-line caregivers and volunteers.

Will my donation stay in my community?

Yes: In most cases your donation is used within your own community. A local Foundation Committee made of hospice professionals, front-line caregivers and volunteers evaluates the extraordinary needs or requests of patients and their families, ensuring that funding is directed to the most critical needs in a timely fashion.

How can I make a donation?

The Foundation is supported through donations made by Cash, Check, or Credit Card: Please make checks payable to Seasons Hospice Foundation or you can also donate with a credit card by phone 847-692-1000 or on the donation page of this website. We accept Visa, MC, American Express & Discover. Donations can be made in memory of or in honor of someone you love and the family of the deceased or honoree will receive notification that a gift has been made (no amount will be listed). Patient families also make donations in honor of a professional caregiver who has provided outstanding care in their loved one’s time of need, or in support of hospice and the Foundation's mission.

How do I make a donation to the hospice in memory of someone?

A memorial gift may be made online by clicking the "Donate Today" link in the left hand menu or on the dove in the upper right corner of the site.

If you are paying by check, simply note on the memo line of your check (or on an attached note) that your gift is "in memory" or "in honor" of an individual. If you wish an acknowledgement letter to be sent to a particular family member or friend of the deceased or honoree, please provide that person’s name and address. Do you accept gifts "In Kind"?

Yes: Goods or services are always appreciated. Please contact Marisa at our National Foundation office at 847-692-1000 x 1063.

Is my donation tax deductable?

Yes: Seasons Hospice Foundation is separate from Seasons Hospice & Palliative Care. The Foundation oversees the disbursement of funds that are donated, bequeathed, or acquired through fundraising events for the organization.

Seasons Hospice Foundation has IRS 501 (c) (3) non-profit status. Donations given to Seasons Hospice Foundation are tax deductible.

How can I volunteer?

Just email us at info@seasonsfoundation.org or call us at 847-692-1000. There are many ways to volunteer at Seasons Hospice. You could volunteer to be part of an event committee, participate in soliciting in-kind donations as raffle prizes, make sponsorship calls, or help out with administrative tasks in the office, to name just a few.

Does Seasons Hospice Foundation help cover costs such as co-pays, out-of-pocket expenses like deductibles or out-of-net-work patients?

The Seasons Hospice Foundation mission is to assist patients and their families who have unique needs that fall  outside the Hospice Benefit. The Foundation provides support for extraordinary needs related to the emotional support and comfort of the patient and their family. Traditional co-pays, out-of-pocket expenses and deductible costs are not expenses that qualify for a Foundation intervention. Does Seasons Hospice Foundation pay for un-funded patients?

Seasons Hospice Foundation interventions do not include covering the cost of  medical expenses. The Foundation does cover the unique needs of hospice patients and their families that fall outside the traditional Hospice Benefit.
